Please Note: In order to self-enroll for the WMH Patient Portal your email address must be on file at WMH. You can submit your email address by printing off the form found HERE and email, fax, or drop off at Medical Records or any WMH practice or Registration. Please call 906-635-4492 for further assistance.
For your convenience, War Memorial Hospital offers a Patient Health Portal - an online tool that goes a step beyond simply displaying information by helping you manage your healthcare easily and securely. This tool is available to all adult patients and minor patients’ parents/legal guardians from the ages of 0-9 years of age.

Health Information Exchanges
A Health Information Exchange (HIE) is a way of sharing your health information among participating doctors’ offices, hospitals, laboratories, radiology centers and other health care providers through a secure, electronic means. This form is to be used by patients who do not want to participate in the Upper Peninsula Health Information Exchange (UPHIE). If you elect to opt-out, your treating providers will not be able to access your health information through the UPHIE system.
